The team consists of pathway advisers and social workers providing support on a range of issues including accommodation, financial support, entry into employment or training, offending and emotional wellbeing.

Prospects’ Leaving Care Service ensures that young people leaving care after having been looked after by the Council  have their needs assessed and have appropriate plans in place. We help with their preparation for transition from care to independent living including creating opportunities to help them strengthen their resilience.

We work closely with carers and colleagues in Children’s Social Care, Young People’s Services and Adult Services resulting in creative, effective and robust Pathway Plans in which care leavers have been involved. We help young people plan their education, training and employment, find suitable accommodation and help them negotiate access to adult and main-stream services including appropriate support funding.

Prospects' pathway workers promote healthy living and emotional wellbeing, signposting the young person to appropriate help and we support them to maximise their life chances by encouraging active engagement in sports, developing special talents, hobbies and involvement in their local community.
